Flower Home Health (FHH) maintains the highest quality home health services, ensuring the quality control of all members of our professional staff in order to achieve the highest standards of home health care.
FHH provides comprehensive, multi-disciplinary nursing rehabilitative therapy, dietary teaching and social services.
We recognize the value and importance of patient and family participation in health care planning and encourage decision-making in a mutual and respectful environment in order to meet the needs of the patient.
FHH is committed to maximizing the rehabilitative potential of each patient while preserving their dignity and independence in the comfort of the home environment.

The Benefits of Home Care Service
Home Care Services can:
- Provide health care in the familiar and comfortable sorrounding of home thereby contributing to the healing process both physically and emotionally.
- Restore health and contribute to patient well-being.
- Prevent costly hospital admissions and reduce incidents of re-admissions.
- Facilitate early hospital or skilled facility discharge.
- Provide continuity of physician care.
Eligibility Criteria
Qualifying clients must:
✔ be under the care of a physician
✔ be homebound and unable to leave home unassisted
✔ require skilled nursing care and/or rehabilitative services
✔ have an established plan or care by a physician
